What is the generic name (s)?

Main ingredients of Melacare are:

  • Hydroquinone[1] Topical : 2% w/w
  • Mometasone Furoate[2] Topical : 0.1% w/w
  • Tretinoin[3] Topical : 0.025% w/w

How does it work ?

The working of the individual ingredients of Melacare are being described below:

Hydroquinone has a reversible depigmentation[4] effect on the skin. The accumulation of Melanin[5], the skin pigment which causes skin darkening, is reduced by its bleaching effect. The synthesis of Melanin along with some important processes which result in the formation of Melanin inside the cells get inhibited by hydroquinone.

Mometasone furoate is a glucocorticosteroid (type of corticosteroid[6]) used topically to reduce inflammation and allergies of the skin or in the airways, by inhibiting the formation of substances which cause them. It circulate easily in the blood, crossing cellular membranes and binding with cytoplasmic receptors. This results in the transcription and synthesis of protein. It interferes with adhesion of leukocytes[7] to capillary walls, the reduction of the permeability of the capillary membranes and consequently edema.It also liberates hydrolace acids of leukocytes and this results in decreased inflammation.

Tretinoin belongs to a class of drugs called retinoids[8] (retinoic acid). It is a form of Vitamin A[9] that helps skin renew itself. It is commonly used topically to treat acne vulgaris[10]. Its main function is to dampen the rate of growth of ill blood cells.

What are it’s uses ?

Melacare is used for its gradual bleaching effect in the management of hyper pigmented conditions of skin such as:

  1. Freckles
  2. Melasma
  3. Senile lentigines and other unwanted areas of melanin hyper pigmentation

How effective is this medicine ?

There are no studies conducted to prove the effectiveness or efficacy of Melacare for skin lightening and anti-ageing , but the claimed individual ingredients can be quite effective in pigmentation reduction and protection against inflammation (which causes wrinkles and freckles and thus ageing of skin).

What are the Side Effects ?

Some commonly reported side-effects of individual generic components of Melacare are being described below:

Hydroquinone : Some of the side effects of hydroquinone include:

  • skin redness due to allergic reaction
  • Itching
  • Soreness
  • Mild skin irritation
  • Inflammation, burning
  • Erythema & stinging
  • It has been linked to ochronosis (skin darkening and disfiguration)

This product has a weaker formula i.e., 2% compared to 4% in the prescription creams like Lustra, Tri-Luma, and EpiQuin Micro, so it is relatively safe when used for short duration.

Mometasone furoate : Some of the side effects ( though these generally last for a short time) of Mometasone furoate include:

  • Changes in color of skin
  • Burning
  • Acne
  • Itching
  • Dryness in the skin
  • Stinging

Tretinoin: Some of the side effects of Tretinoin are :

  • Facial swelling
  • Blisters at the cream applied area
  • Skin redness ( especially facial redness)
  • Darkening of the color of the skin
  • Itching, causing thickening of the skin
  • Tenderness in the skin
  • Scaling and mild burning
  • Reaction at the cream applied area
  • Worsening of the acne in first 2 – 4 weeks ( this happens in the beginning, when you start applying Tretinoin on your skin but this effect dampens down with continued usage)
  • Dryness in the skin (hence, it is suggested that you use a good moisturizer in the day time)

Is Melacare addictive?

Habit forming tendencies for this medicine has not been reported. Most medicines don’t come with a potential for addiction. What happens if Melacare over-applied and how to manage?

It is not recommended to use more than the prescribed dose. Taking more than the prescribed dose will not improve your symptoms,  it may cause serious side-effects. Can Hydroquinone cause cancer?

Reportedly there are no cancer cases in human beings linked to hydroquinone till date, so weaker strength (0.5% to 2%) Hydroquinone is still available in some of the over the counter products, while stronger (4%) Hydroquinone is only available on prescription. Hydroquinone has been on cancer watch list of FDA and it’s usage has been quite controversial, especially because life time usage of hydroquinone on animals has been linked to increased risk of cancer due to ochronosis.
